Cory Adam Frank, 31 of Fall Creek, WI passed away Wednesday, June 6, 2018 at Mayo Clinic Health Systems, Eau Claire.

A Celebration of Cory’s life will be held from 2 – 4 p.m. Saturday, June 16, 2018. Military Honors will be conducted at 4p.m. at Stokes, Prock & Mundt Funeral Chapel 535 Hillcrest Pky. Altoona Wi.
